- Upon admission, and with a comprehensive assessment of your rehabilitation goals and personal needs, we create a personalized comprehensive rehabbing care plan in conjunction with your physician and your family.
A PERSONALIZED PROGRAM
- Whether you are recovering from surgery or illness, our innovative programs effectively deliver optimal short-term rehabilitation outcomes in Orthopedic Recovery, Pulmonary Rehabilitation, Stroke Management and Cardiovascular Care.
- Utilizing the latest clinical protocols and rehabilitation technology, our stellar interdisciplinary team of therapists, nurses and doctors ensures an all-encompassing continuum of care focused on every patient’s physical, social and emotional well-being.
- Intensive Physical, Occupational and Speech Therapy are delivered up to 7 days per week to restore mobility and improve strength, stamina, endurance and communication skills as deemed appropriate.
DISCHARGE
- When you are ready for discharge, our personalized discharge protocols ensure a smooth transition back into your home environment. Whether it is meticulous guidance as to how to perform your favorite daily routines or arranging for assistive devices, our rehab specialists will go over your concerns and proactively address any issues that could impact your successful return home.
- Follow-up with your primary care physician will likely be an important part of your ongoing recovery. For this reason, we will proactively schedule your initial follow-up appointment prior to your discharge and communicate directly with him/her to ensure an optimal care transition.
- Our care for you continues into your post discharge recovery. To ensure your continued well-being, our team of professionals will reach out to you to check in. Avoiding unnecessary rehospitalization is our top priority.
