Jeppesen, MD, MBA SFHM, Subacute Medical Director, provides specialized care and on-site hemodialysis service to patients with chronic kidney disease (CKD) and related health problems. <\/p>\n<p>For seniors with advanced renal disease, the frequency of trips back and forth to a community dialysis center can be exhausting. Fortunately, at The Brentwood Rehabilitation and Health Care Center, the convenience is just a few steps away. <\/p>\n<p>Our state-of-the-art in-house Dialysis center features a 4 Chair Hemodialysis Unit In Partnership With DaVita. <\/p>\n<h3>Chronic Kidney Disease Treatment Program aims to:<\/h3>\n<ul>\n<li>Reduce Symptoms for people with CKD<\/li>\n<li>Decrease Disability <\/li>\n<li>Increase Quality of Life <\/li>\n<li>Promote Independence &#038; Physical Activity <\/li>\n<\/ul>\n<h3>Patients &#038; family members benefit from:<\/h3>\n<ul>\n<li>Onsite Hemodialysis <\/li>\n<li>Consultative Visits by On Staff Nephrologists <\/li>\n<li>Health Coaching <\/li>\n<li>Dietary Education <\/li>\n<li>Management &#038; Education for people with Diabetes <\/li>\n<li>Routine Medical Management of Associated Co-Morbidities <\/li>\n<li>On-Site Community CKD Monthly Support Group Facilitated by Community CKD Experts <\/li>\n<li>Case Management Services <\/li>\n<\/ul>\n<p>Preserving kidney function and preventing further kidney damage are the primary objectives of our Chronic Kidney Disease Management Program.
